Providing Transportation Assistance
Not having a vehicle or being unable to drive should not prevent you from accessing quality healthcare services. We can help you travel to and from your appointments through our transportation assistance services.
The Blackhawk Health Center has three drivers. You may request transportation assistance if you are eligible for services at our health center or Purchased/Referred Care.
Scheduling Transportation
To schedule transportation, you must contact the Blackhawk Health Center at least three (3) business days in advance.
Your appointment must already be scheduled at the Blackhawk Health Center, Merle Boyd Activity Center, or Community Health Department for IADLs (Independent Activities of Daily Living). You may also schedule transportation if you have been referred through Purchased/Referred Care for a medical, dental, or eye appointment.
Please note that transportation is not guaranteed, and scheduling is based on availability.
