The Blackhawk Health Center was founded in 2022 to provide high-quality care to patients,
share essential health information, and host trainings that support life-long wellness.
Our center includes a state-of-the-art lab, a private gym, a walk-in and drive-through pharmacy,
and a kitchen for cooking demonstrations. These all support our wide range of healthcare services,
allowing us to provide comprehensive, whole-person care.

To provide excellent healthcare services that impact and improve the health and wellness of members of the Sac and Fox Nation, other Indigenous Nations, and the surrounding communities.
Who We Serve
The Blackhawk Health Center strives to provide the best possible care to our community. Our patient registration is open to any Native American patient recognized by a federally recognized tribe, living within our jurisdictional area.

Blackhawk Health Center Staff
Our center’s staff includes a variety of departments, all working together to provide the best care to our patients and enhance the health and wellness of our community. Staff members you may meet with include the:
- Business Office staff.
- Patient Registration Team, which includes a telephone operator and a Benefits Coordinator.
- Health Information Management team, which includes 2 information clerks, 1 scanner, 2 medical coders, and 1 health information manager.
- Medical Department, which is led by an M.D. and staffed with 3 full-time medical providers.
- Nursing Department, which is led by a Registered Nurse with 4 Licensed Practical Nurses.
- Community Health Workers, which is led by a Registered Licensed Nurse with 2 Licensed Practical Nurses and 2 Certified Nurse Assistants.
- Lab Department, which is led by a registered MLT.
- Radiology Department, which is led by an AART Registered Radiographer.
- Behavioral Health Services Department.
- Physical Therapy Department, which is led by 2 full-time physical therapists.
- Nutrition Department, which is led by a Licensed Registered Dietitian.
- Podiatry Department, which is led by a board-certified podiatrist.
- Special Diabetes Program staff.
- Contract Health Services Department.
- Transportation Department, which is staffed by 3 drivers.
