Call to schedule an Emergency Appointment if you are in severe dental pain.

To fill out your paperwork ahead of time, please print the Heath History and bring with you.

NOTICE: The Dental Clinic has a separate entrance from the main clinic. Please enter on the northwest corner of the building.
