We aim to provide quality care services directly to our patients. Our Community Health Department facilitates healing and supports our community through outreach health services, home visits, and patient advocacy.

Our Community Health Workers provide a range of quality outreach health services. This includes:

  • Sharing information and resources that help improve health.
  • Providing disease prevention services.
  • Implementing medical protocols to facilitate healing.
  • Identifying and assisting with unmet physical and psychosocial needs.
  • Providing emotional support to patients.
  • Acting as a patient advocate.

What to Expect During Your Home Visit

For your appointment, you will meet with a Licensed Practical Nurse and/or Certified Nurse Assistant. During the visit, your nurse will provide important care services, like checking your blood pressure, helping with medication refills, changing wound dressings, and more. Throughout this time, and beyond your visit, they will provide emotional support and advocate for you and your health.

About the Community Health Department

Our Community Health Workers (CHWs) aim to bring quality healthcare services directly to patients in our community. CHWs aim to support the physical, psychosocial, and emotional health of patients while promoting overall health and preventing disease.

The Community Health Department is led by a Registered Licensed Nurse (RN). The team also includes two Licensed Practical Nurses (LPNs) and two Certified Nurse Assistants (CNAs).
